Applicants applying for an original or renewal Florida driver license must provide proof of citizenship, residential address, proof of social security number and proof(s) of name change from birth, if any. Non US Citizens must present proof of legal presence.  For further information please visit Gather Go Get. All applicants must show proof that they have completed a traffic law and substance abuse education course and will be required to pass vision and hearing tests, road rules and road sign tests, plus a driving test.

Originals with valid out of state driver license and ID card-United States Citizens

Florida law requires proof of citizenship or proof of legal presence,  two proofs of residential address, proof(s) of name change since birth and proof of Social Security Number (if issued, Chapter 322, Florida Statutes, requires the Department see proof of social security number for the issuance of driver license and identification cards) from all customers before a driver license or identification card can be issued. The name assigned to the social security number must match the name that will appear on the Florida driver license or identification card. If you have recently changed your name, update your records with the Social Security Administration before you apply for your license or identification card. Each U.S. citizen who applies for an original driver or replacement license or identification card must submit:

  1. Proof of Residential Address
  2. Proof of Social Security Number
  3. Primary Identification

Changed Your Name?

A name change  requires the issuance of a new Driver License or Identification Card. A change of name cannot be processed by mail or Internet. A replacement with new name and photo may be obtained at our office. The following documentation is required:

United States Birth Certificate OR valid United States Passport. A birth certificate must be issued by a government agency. Hospital birth certificates cannot be accepted.

Social Security Card. Upon changing your name, update your records with the Social Security Administration, wait 24 hours to update before you apply for your Florida Driver license or Identification Card.

Proof of residential address requires two of the following reflecting your name and address: deed, mortgage statement, lease agreement,voter registration, vehicle registration, W2 form, 1099 form, or a homeowners, automobile, or life insurance policy. If the documentation submitted reflects any name change since birth, an original or certified copy of your Marriage License(s), Divorce Decree or Court Order,must be submitted. In some instances, more than one marriage license, divorce decree or court order document may be required to show the chain of name changes. Photocopies cannot be accepted. A change of name cannot be processed by mail or Internet.

Note: Giving false information on an application for a driver license or identification card is a third degree felony which may result in arrest, and includes a one-year suspension of the driving privilege. Proof of correct address may be required, for example, cancelled mail envelope received.
