Human Resources: Employee Benefits Enrollment

 

Hernando County Employee Benefit Enrollment Services are provided by BenTek.

Employees can access the BenTek web site 24 hours a day / 7days a week, to review benefit selections and make changes during the open enrollment period.

Benefit eligible employees will be automatically enrolled in the following core coverage's:

  • Basic Life & Accidental Death & Dismemberment (AD&D) Insurance: $10,000 Benefit  Provided by Principal
  • Employee Assistance Program
    Provided by The Hartford: Customer Service:  1-800-96-HELPS (1-800-964-3577).
  • Short/Long Term Disability Insurance
    Provided by The Hartford

Health Insurance
The County offers four health insurance plan options provided by BlueCross BlueShield of Florida (BCBS of Florida has now re-branded their name to “Florida Blue”).

  • Blue Options Plan #03564 (formerly 1767 Plan)
  • Blue Options Plan #03748 (formerly 1559 Plan)
  • Blue Options Plan #5190/5191 (formerly 1160-1161) High Deductible Health Plan w/ HSA account
  • BlueCare HMO Plan #042 (formerly Plan #13)
     

Health Savings Account
Employees who elect coverage in the #5190/5191 (formerly 1160-1161) are eligible to open and contribute to a Health Savings Account (HSA) provided by Mellon Bank. Note: The County funds each HSA $1,000 each plan year. This is in addition to any voluntary amount you elect.

Flexible Spending Accounts
The County offers two Flexible Spending Accounts (FSA) plan options provided by AFLAC/FlexOne:

  • Health Care FSA
    • Minimum Contribution: $5.00 per pay period
    • Maximum Contribution: $75.00 per pay period
  • Dependent Care FSA
    • Minimum Contribution: $5.00 per pay period
    • Maximum Contribution: $200.00 per pay period

Dental Insurance
The County offers three dental insurance plan options:

Vision Insurance
The County offers vision insurance provided by EyeMed and VSP Vision Care.

Supplemental Life & Accidental Death & Dismemberment (AD&D) Insurance
The County offers supplemental life insurance provided by the Principal.

  • Benefit amount can be elected in increments of $10,000 to a maximum of $190,000
  • Benefit election cannot exceed 6 times your annual salary

Dependent Life Insurance
The County offers dependent life insurance provided by the Principal. Benefit eligible employees can elect dependent life insurance for their spouse and / or child(ren) for $2,500 or $5.000. Dependent Life Insurance may also be elected in the amount of $10,000 if your total Basic Life / AD&D and Supplemental Life / AD&D benefit amount equals $20,000 or greater.

Short Term Disability Insurance
The County offers short term disability insurance provided by MetLife.

Pre-Paid Legal Plans
The County offers two pre-paid legal plan options provided by Pre-Paid Legal:

AFLAC
